There are various forms used at the Traffic Violations Bureaus.  Below is a partial list of these forms:

Request For Driving Record Or Registration Information - Use this form to get a copy of your New York driving record or registration information.   Alternatively, and without delay, contact our office to obtain a copy of your driving record. 

TVB Appeal Form - Use this form to appeal a guilty conviction or penalty of any TVB traffic ticket (NYC, Hauppauge, Buffalo or Rochester).  TVB Appeal Forms must be sent within 30 days of your conviction date. 

Bond Refund Form - Use the Bond Refund Form to receive a refund of your $40 bond payment after you have won your case.

Sample Abstract Of Driving Record - Use this form to to see a sample New York Driving Record Abstract including an explanation of codes and list of abbreviations. 

DMV Hearing Appeal Form - Use a DMV Hearing Appeal Form to appeal a finding and/or penalty imposed after a DMV hearing regarding such matters as a fatal accident, chemical test refusal, or motor-vehicle related business regulated by DMV.
